We are hosting an admissions event for the 2024/2025 school year. Come to one of our upcoming Tuition Assistance & Tours event so that you can learn more about JLCCS, take a tour, and apply for tuition assistance. At this event, we will assist families in applying through FACTS for HALO assistance.
Events will be held in the school on:
- February 25th from 10am to 2pm
- March 5th from 6pm to 7:30pm.
To complete your FACTS application, you will be required to submit the following documents, so please bring these documents:
❏ Copy of 2023 IRS Federal Form 1040, 1040A or 1040-EZ U.S. Individual Income Tax Return. If applicant and co-applicant file separately, we require both tax returns for the same tax year.
❏ Copies of all 2023 W-2 Wage and Tax Statements for both the applicant and co-applicant. NOTE: If you are applying before you have received all the 2023 W-2 Wage and Tax Statements, please submit them as soon as they become available.
❏ Copies of all supporting tax schedules if you have income from any of the following:
• BUSINESS – (Form 1040 Line 12) Attach Schedule C or C-EZ and Form 4562 Depreciation and Amortization
• FARM – (Form 1040 Line 18) Attach Schedule F and Form 4562 Depreciation and Amortization
• RENTAL PROPERTY – (Form 1040 Line 17) Attach Schedule E (page 1) • S-CORPORATION – (Form 1040 Line 17) Attach Schedule E (page 2), Form 1120S (4 pages), Schedule K-1 and Form 8825
• PARTNERSHIP – (Form 1040 Line 17) Attach Schedule E (page 2), Form 1065 (5 pages), Schedule K-1 and Form 8825
• ESTATES AND TRUSTS – (Form 1040 Line 17) Attach Schedule E (page 2), Form 1041 and Schedule K-1
❏ If you do not file income taxes and are unable to furnish a tax return or other tax schedules, you must provide copies of supporting documentation for Taxable Income, such as a letter from your employer.
❏ Copies of all supporting documentation for household Non-Taxable Income including: Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and Temporary Assistance for Needy Families (TANF).
NOTE: You must provide documentation to verify the source of any wages/income listed on your application. Applications that do not include this supporting information are incomplete and will not be considered for tuition assistance.
Additional Items to Bring:
- Monthly rent or mortgage payment (for all properties if you own more than one home)
- Total credit card debt and your minimum monthly payment on your credit cards
- Other loan amounts, including student loan balances and payment amounts
- Alimony payments & child support payments
- Amount of your monthly health insurance payment
- Annual medical expenses not paid by insurance
- Year, make and model of all vehicles.
- Monthly car loan payment amounts
- Annual vehicle insurance expense
- Charitable donations in cash or check.
- Value of all 529 plan accounts
- Value of retirement plan assets
- Annual amount spent on summer vacations and activities for ALL children
- Value of cash, savings accounts and checking.
- Value of all investments: stocks, bonds, cryptocurrencies, mutual funds, certificates
- Spouse's annual contribution to retirement plans
- Estimated value of your home if you own it
- Amount you owe on your mortgage
- You will need your child's social security number.
